The Meeker Family Health Center is a vital part of Pioneers Medical Center, providing a wide range of healthcare services to families in the White River Valley. Our family health services provide important treatments for patients of all ages, from prenatal to senior stages of life.

We are federally certified as a Provider-Based Colorado Rural Health Clinic and accept most insurance plans. Convenient appointments are available Monday through Friday from 8 am to 5 pm and Saturdays 9 am to 12 pm.  To make an appointment, please call (970) 878-4014.

Our comprehensive primary care treatment encompasses several different aspects of your healthcare:

Wellness and prevention services are offered to keep you healthy and free of disease. These services include:

  • Regular screenings and checkups to detect health issues early
  • A careful review of your health history to identify any risk factors that need to be monitored

  • Lifestyle change recommendations that will help reduce your risk of illness in the future
  • Education related to disease prevention and treatment

Acute healthcare covers injuries or illnesses that develop suddenly, but typically don’t have a long-term impact on your overall health. This may include common illnesses such as a cold, sore throat or the flu. It also involves treatment of injuries, such as sprains, which typically don’t require treatment from a specialist.

Your primary care physician will also help you manage chronic, long-term conditions such as asthma, diabetes or high cholesterol. Your doctor will work with you over an extended period of time to monitor the condition, track any changes that occur and decide when changes to your treatment plan are needed.

Coordinated Chronic Care Program

Chronic Care Management is designed to help connect patients to the many resources they need to manage their health. If you have Medicare and live with two or more chronic conditions like arthritis, diabetes, depression or high blood pressure, then this program is perfect for you.

To get started, you will need to be an established patient having your primary care provider at Meeker Family Health Center. This will ensure that all your information and medical history is up to date. Once you are enrolled in the program you will find that you have great access to coaching and care resources, including:

What if I am not on Medicare?

If you are over age 65 or have been eligible for Social Security Disability benefits for at least 24 months, then you may be eligible for Medicare.

You can enroll in Medicare Part A and/or Medicare Part B in the following ways:

  1. Online at Social Security (SSA);
  2. By calling Social Security at 1-800-772-1213 (TTY users 1-800-325-0778), Monday through Friday, from 7AM to 7PM.

Once enrolled in Medicare Part B you will have up to 12-months from enrollment to have your “Welcome to Medicare” preventative visit. This is like an annual physical and will help to make sure your medical records, family history, prescriptions are accurate.

Walk-In Visits

The Meeker Family Health Clinic offers walk-in visits for patients in need of the following services:

These visits must be done when we have a provider available in order to ensure we can provide the necessary treatment in the event of an emergency from an adverse reaction.

Walk-in visits are available at the following times (except holidays):

With the exception of the flu shot, walk-in visits are for established patients only. If you haven’t been seen by our clinic in the past, you must make an appointment to establish care before taking advantage of our walk-in visits.
