2002-23 Vaccination Waiver for Meeker School District

If your family has received communication from the Meeker School District regarding a new, annual vaccination exemption form for the 2022-23 school year, Pioneers Medical Center is here to help. 

For our providers to sign your child’s vaccine exemption waiver, the following must occur:

1. Your child must have had a well-check appointment within the past year. You must schedule a well-check appointment if your child has not had one within this timeframe. Sports Physicals are not well-checks. 
2. Your well-check appointment and/or consult appointment to obtain a provider-signed exemption form will include a vaccination education component with your provider.
3. This does not pertain to the Flu Shot or COVID vaccine.

Suppose you prefer not to schedule a well-check or appointment with a PMC provider. You may visit the Colorado Department of Public Health and Environment’s (CDPHE) website for an online waiver option by following the instructions below:

1. This is the link that outlines the exemption process: cdphe.colorado.gov/vaccine-exemptions.
2. Scroll down to the middle of the website.  You will see an arrow with “Online Immunization Education Module.” Click on this link.
3. The module is 20 minutes long, and at the end, you will be able to fill out the exemption form (if you have multiple children, you only need to do the module once, but fill out multiple forms at the end of the module). You can print the form or press the button to upload the document to CIIS (Colorado Immunization Information System).  (If you choose to upload it, I will be able to see it in the system immediately.)
4. It is not recommended to use a cell phone, as other parents have had difficulty with the module at the end when using it.
