The Meeker Family Health Center is a vital part of Pioneers Medical Center, providing care from prenatal to senior stages of life. The convenient location and skilled medical team allow area residents, workers and visitors access to primary care, minor office procedures and appropriate testing close to home, work and play, helping to eliminate timely and cumbersome trips to outlying facilities. Meeker Family Health Center is proud to be the medical home for many families in the White River Valley.
The center features 12 clinic exam and procedure rooms, 2 designated specialty exam rooms, and a children’s area with 3 pediatric-specific exam rooms. As always, the Meeker Family Health Center can be your “medical home” – a partner to help you navigate the complexities of healthcare. Our team proactively collaborates with trusted specialists to refer patients or coordinate follow-up care in Meeker. This ensures uninterrupted care, better communications and smooth transitions between care providers.
We are federally certified as a Provider-Based Colorado Rural Health Clinic and accept most insurances. Convenient appointments are available Monday through Saturday and in the evenings for specialty clinics. To make an appointment, call (970) 878-4014.
The Meeker Family Health Center provides services such as:
- Adult wellness check-ups and preventative care
- Women’s health services, including GYN exams, prenatal care and health screenings
- Pediatric care, immunizations and well-child exams
- Chronic disease management, including treatment of diabetes and asthma
- Home health care services
- Minor office procedures, e.g. mole removal
- DOT physicals
- Geriatric medicine
- Treatment of common illnesses and injuries
- Industrial medicine and workplace wellness programs like Trails to Health
- Workers compensation exams
- Aesthetic /cosmetic services

With a background in public health and the aging population, Janelle has found passion in fostering independence and health promotion among the Medicare chronic care population. Bridging the gap between community resources and preventative medicine is key in assuring our patients live the quality of life they desire. What is Chronic Care Management?
Chronic Care Management is designed to help connect patients to the many resources they need to manage their health. If you have Medicare and live with two or more chronic conditions like arthritis, diabetes, depression or high blood pressure, then this program is perfect for you.
To get started, you will need to be an established patient having your primary care provider at Meeker Family Health Center. This will ensure that all your information and medical history is up to date. Once you are enrolled in the program you will find that you have great access to coaching and care resources.
- 24-7 access to care, including telephone, and other non-face-to-face means of communications (emails, or web-based tools)
- Scheduled routing appointment with your regular primary care provider and care team to create your personalized care plan
- Coordination of care for prescription refills, medication reconciliation, and medication management
- Management of care as you navigate between various care providers and specialists or healthcare facilities.

What if I am not on Medicare?
If you are over age 65 years, or have been eligible for Social Security disability benefits for at least 24 months, then you may be eligible for Medicare.
You can enroll in Medicare Part A and/or Medicare Part B in the following ways:
- Online at Social Security (SSA);
- By calling Social Security at 1-800-772-1213 (TTY users 1-800-325-0778), Monday through Friday, from 7AM to 7PM.
Once enrolled in Medicare Part B you will have up to 12-months from enrollment to have your “Welcome to Medicare” preventative visit. This is similar to an annual physical and will help to make sure your medical records, family history, prescriptions are accurate.
