Pioneers Medical Center offers community education, events and medical screenings as part of our dedication to helping people live longer, healthier lives.

The Meeker Family Health Center is a vital part of PMC, providing care for wellness and preventive services. Meeker Family Health offers a wide range of health programs to ensure you maintain optimal levels of health. These programs are an important component delivering the care you need.

Trails to Health

We’ve created our Trails to Health program to offer you a broad spectrum of preventive health screening services in order to provide early detection and treatment of diseases. As part of this program, our team will work with you to identify opportunities that will help you maintain an active, healthy lifestyle.

Once you’re registered in the Trails to Health program, you will receive reminders about necessary examinations and immunizations. PMC will contact you periodically to support your progress towards personal health goals.

Respect Your Choice

We also understand that some individuals may be uncomfortable with certain parts of a physical examination. You may opt out of any portions of the exam you’re uncomfortable with. We’ll explain the reason we recommend this particular examination to ensure you understand its importance, but we will always respect your choice to opt out.

You’ll receive a variety of important preventive screenings and exams as part of the Trails to Health program. The specific services involved vary based on your age and gender.

Women Under Age 50

Quit Smart

Call the Colorado QuitLine at 1-800-QUITNOW (1-800-784-8669) or visit their website for free support from a Quit Coach and free nicotine patches. You’ll receive a personal quit plan from an experienced Quit Coach, plus tips and support that increases your chances of quitting for good.

Free Nicotine Patches

A free supply of nicotine patches that control your cravings for tobacco will be mailed directly to you. In order to receive free nicotine patches, you must participate in the Colorado QuitLine program and be at least 18 years old.

Free Self-Help Guide with Quit Tools to Help You Be Tobacco Free

This guide includes lots of information, techniques and tools employed by many tobacco users who have successfully quit. Learn great ways to cope with stress and create a happier, healthier lifestyle while you are quitting and after your quit date.

Foot Clinics

Pioneers Home Health Services offers monthly foot clinics for the following patients:

  • Blind
  • Disabled
  • Individuals over the age of 75

Monthly foot clinics are available at the Meeker Veterans Community Center on the second Wednesday of each month.

The foot clinic is conducted by a Certified Nurse Aid and includes:

  • Cleaning feet
  • Trimming toenails
  • Providing skin care

Cost is $20. Financial assistance is available (call for details). Please call 970-878-9265 for additional information or to schedule an appointment.
