Position Title:
REGISTERED NURSE, HOME HEALTH

Position Description:
Home Health RN is responsible for planning, coordinating, providing and documenting care for assigned caseloads of patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the Case Manager RN develops and implements a home health plan that meets each patient’s specific needs and is in compliance with federal and state regulations, reimbursement

Qualifications:
Education:
Graduate of an accredited school of nursing. BSN preferred.
Experience:
Home Health nursing experience preferred.
Physical Requirements:
Requires sitting, standing bending, lifting, pushing, pulling, and moving intermittently during working hours.
License Required:
Current Colorado licensure as a Registered Nurse.
PT position - eligible for benefits.
