A Personalize Care Plan
 Meeker Family Health Center is launching Trails to Health, a comprehensive program that will transform the old-fashioned physical. This program will streamline your annual check-up into a plan customized for your health needs.
Together, you and your doctor/medical practitioner will design a healthcare partnership that set goals and tracks your progress. It is offered at the time of your annual physical and includes screening tests, exam and consultation bundle into one affordable package.
“We are excited to offer an affordable healthcare package that offers the screenings and tests for the primary health indicators,” says Dr. Albert Krueger. “This service has been designed to provide optimal care in a format that mirrors the value of the 9 Health Fair.”
The process is simple. When you call to make your annual check up appointment you will complete a questionnaire to review your current medical history. From your questionnaire answers, your provider will recommend the appropriate tests or screenings you need.
You come to Pioneers Medical Center to have those tests and screenings completed. When the results are in you will be contacted to set an appointment for your physical examination.
At this appointment you will review your results together and develop a plan to improve or maintain you health. Then you will receive personal follow up throughout the year to ensure your health plan is working for you.
