While promoting independence, our trusted and competent clinicians provide oversight and care for you or your loved one in a professional and compassionate manner.
Pioneers Hospital Home Health team works with you and your physician to meet your needs during an illness or while you are recovering from an accident or surgery. Home health can be an alternative, to the nursing home or assisted living center. We provide the following care to assist individuals and/or families to promote independence and maximum their abilities while in the home:
Skilled Nursing
Registered nurses provide:
Necessary assessments (medical history, physical status, home environment and safety, day-to-day abilities)
Coordination of services among all caregivers
Creating and monitoring a plan of care with the assistance of the client, family, physician, etc.
Education for the client and/or family regarding disease management, medications and medication management, IV therapy, wound care, catheter/ostomy management, and injections to name a few
Nurse aide supervision
Certified Nurse Aides
Home care aides under the supervision of a registered nurse provide:
Personal care and hygiene
Ambulation and exercises
Assistance with many activities of daily living
Physical Therapy
Registered physical therapists provide:
Restorative and rehabilitative services
Assistance with pain control
Education regarding exercise programs, gait training, safe ambulation in the home and fall prevention
Modalities and neuromuscular re-education to improve strength, coordination and balance
Personal Emergency Response Systems
This product allows direct access to the 24 hour on-call response center for emergency assistance via a phone line. A lightweight waterproof personal activator is worn around the neck or wrist provides a peace of mind for clients, family members, loved ones.
Medication Alarm Boxes
Reminds and dispenses meds in accordance with prescribed drug regimens. Medication boxes are filled and organized by a licensed nurse and/or family members taught how to fill.
Community Services
Monthly blood pressure clinic – 2nd Wednesday of every month at the Chuckwagon meal
Monthly foot clinic - Last Tuesday of every month, alternating between the Fairfield Center (200 Main St.) and The Pines apartment (675 Water St.)
We are certified to provide care through Medicare, and Medicaid. Therefore, we accept payment through these organizations, commercial insurance companies, and private pay.
